Audit and Verification of Services
Because Raya Health bills payers for services rendered and is responsible for the quality and integrity of care delivered through its platform, Raya Health reserves the right to review, verify, and audit records and services at any time. Doulas shall:
- Acknowledge that Raya Health has the right to audit, review, and request any patient record, visit note, documentation, message, or related record at any time, with or without prior notice.
- Cooperate fully and promptly with any audit or review, including providing all requested records, documentation, and information within the timeframe Raya Health specifies.
- Maintain complete, accurate, and contemporaneous records sufficient to support every service documented and billed.
- Retain records in accordance with Raya Health policy and applicable law, and make them available upon request, including after the engagement has ended.
- Permit Raya Health to verify that documented visits and services actually occurred, were delivered as described, were timely, and met Raya Health's standards and applicable requirements.
During audits, specific patients and their visits will be under review. Doulas may be asked to provide proof of a patient visit, and duration of provided care. These records may include:
- Phone call logs
- Virtual call logs
- Record of relevant ride shares
- Patient sign-off sheets
- Session notes
- Text and email confirmations
- Calendar/scheduling records
- Photos of visit
- Other relevant documentation
These records may be delivered in the form of images, digital screenshots, pdfs, video, and other media.
